Connectivity- vs Scalp-Based Targeting of Accelerated Transcranial Magnetic Stimulation for Depression

Does individualized, connectivity-based targeting improve treatment-resistant depression more than scalp-based targeting for high-dose accelerated transcranial magnetic stimulation (aTMS)?In this randomized clinical trial, 40 adults with treatment-resistant depression showed significantly larger antidepressant benefits with connectivity- vs scalp-based targeting of aTMS. Connectivity-based targeting of a previously defined convergent depression circuit had a large effect size, with a number needed to scan of 5 individuals, and connectivity-based targets were reproducible within individuals and different between individuals.The findings suggest that individualized, connectivity-based targeting of the convergent depression circuit in high-dose aTMS should be considered for individuals with treatment-resistant depression.
