Total Thyroidectomy vs Lobectomy for Sporadic Medullary Thyroid Cancer

What are the oncologic outcomes of total thyroidectomy vs lobectomy in patients with sporadic medullary thyroid carcinoma?This systematic review and meta-analysis of 9 studies including 1371 patients found no differences between total thyroidectomy and lobectomy in mortality, overall survival, biochemical cure, and development of distant metastases. Total thyroidectomy was associated with lower odds of structural recurrence, though the difference was not maintained in sensitivity analyses, and lobectomy plus central neck dissection showed a low rate of adverse events.Evidence from these retrospective studies suggests that thyroid lobectomy may achieve comparable oncologic outcomes to total thyroidectomy in patients with sporadic medullary thyroid carcinoma.
