Diagnostic Performance of Prespecified OCT Rules for Glaucomatous Optic Neuropathy in Nonpathologic Myopia

Can prespecified optical coherence tomography (OCT) rules accurately detect glaucoma in eyes with nonpathologic moderate and high myopia (spherical equivalent ≤−3.0 diopters or axial length ≥24.5 mm, excluding staphyloma or severe maculopathy)?In this multicenter diagnostic study of 1525 eyes, a qualitative OCT rule identifying a temporal-superior-inferior-nasal-temporal (TSNIT) curve dip or depression achieved sensitivity of 0.93 and specificity of 0.93 in an external cohort. A quantitative rule using inferior peripapillary retinal nerve fiber layer or inferotemporal macular ganglion cell–inner plexiform layer thinning demonstrated high diagnostic accuracy (0.90 sensitivity, 0.93 specificity).Results indicate these prespecified OCT rules may serve as effective decision-support tools to help clinicians distinguish early glaucoma from physiological myopic variations.
