Evaluating Metrics Assessing Surgical Success in Patients Undergoing Cataract Surgery

What are the use and success rates of Merit-Based Incentive Payment System (MIPS) measure 191 for measuring cataract surgery quality?In this cohort study, among 55 132 patients from 16 health systems in the Sight Outcomes Research Collaborative (SOURCE) consortium undergoing cataract surgery, approximately 9 of 10 surgical procedures met the MIPS measure 191 criteria for success; however, nearly half of patients were excluded from the metric due to ocular comorbidities. Older patients and Black patients (self-reported) were more likely to be excluded.MIPS measure 191 has strengths and limitations, and this study suggests ways to modify this metric to enhance its use and highlights the potential importance of case-mix adjustment when comparing surgeon performance.
