Peri-Event Intravitreal Anti-VEGF and Systemic Outcomes After Stroke or Myocardial Infarction

Among patients receiving intravitreal anti–vascular endothelial growth factor (anti-VEGF) who experience acute stroke or myocardial infarction (MI), is pericardiovascular event (CVE) treatment associated with mortality or major sequelae?In a multicenter electronic health record network cohort, 1526 and 1523 matched pairs with stroke and MI, respectively, were analyzed. Peri-CVE treatment was not associated with a worse mortality or morbidity outcome, and no differences in outcomes among commonly used anti-VEGF agents from 2005 through 2025 (aflibercept, bevacizumab, and ranibizumab) were identified.These findings reveal that peri-CVE anti-VEGF was not associated with harm and may be continued without altering timing or agent selection.
