What is the incidence of acute angle closure (AAC) after pharmacologic pupillary dilation in a large county-wide teleretinal diabetic retinopathy screening program? In this cohort study of 84 008 patients and 168 796 dilations, there were 4 cases of medical record–confirmed AAC. AAC incidence was 2.4 per 100 000 dilations or 4.8 per 100 000 patients. These results support the safety of pharmacologic dilation in primary care–based teleretinal screening programs.
