Circulating Tumor DNA Status and Adjuvant Chemotherapy in Resected Colorectal Liver Metastases

Is postsurgical circulating tumor DNA (ctDNA)–defined molecular residual disease (MRD) status predictive of survival outcomes and benefit from adjuvant chemotherapy (ACT) in patients with resected colorectal liver metastases (CRLM)?In this prognostic study including 298 patients, MRD positivity 2 to 10 weeks after surgery was associated with worse survival outcomes. Among patients undergoing upfront surgery, ACT was associated with improved overall survival among patients with positive MRD but not negative MRD.ctDNA-defined MRD may help refine postsurgical treatment strategies and support further evaluation of ctDNA-guided approaches in resected CRLM.
