Early vs Delayed Extubation After Thrombectomy for Acute Ischemic Stroke

In patients with acute ischemic stroke undergoing endovascular thrombectomy under general anesthesia, does early extubation (<6 hours) improve 90-day functional outcomes compared with delayed extubation (6-12 hours)?In this randomized clinical trial involving 174 patients, functional independence at 90 days was not significantly different among patients in the early extubation compared with the delayed extubation group. No significant differences were observed in overall distribution of modified Rankin Scale score, length of hospital stay, complications, or mortality.Without an apparent association between early extubation and improved functional recovery, clinicians should take an individualized approach to postprocedure airway management for patients with acute ischemic stroke treated with thrombectomy under general anesthesia.
