Can plasma phosphorylated tau 217 (p-tau217) reliably identify amyloid β (Aβ) status in cognitively unimpaired individuals for participant selection in preclinical Alzheimer disease (AD) trials or for potential future use in clinical practice?In this cohort study of 2916 cognitively unimpaired individuals, plasma p-tau217 demonstrated good accuracy in predicting Aβ positivity. However, achieving a positive predictive value above 90% required confirmatory testing with a cerebrospinal fluid or positron emission tomography test.Plasma p-tau217 is a useful stand-alone marker of Aβ status in cognitively unimpaired individuals when moderate accuracy suffices, but adding another test in a second step is needed to achieve a high accuracy in this population.
