Thiazides and Risk of Hyponatremia by Age and Sex

What is the age-specific and sex-specific risk of hyponatremia associated with newly initiated thiazides?In this cohort study of 159 080 individuals, among women 80 years or older, use of thiazide diuretics was associated with a statistically and clinically significant risk of hyponatremia among women aged 80 years or older compared with use of calcium channel blockers. In contrast, among women younger than 65 years, the risk of hyponatremia was negligible.The findings of this study suggest that prescribers should consider close clinical monitoring among older adults, especially women, receiving thiazide, and that alternative antihypertension treatments may be considered in this population.
