Which demographic and histopathologic factors are associated with colorectal adenoma recurrence in postpolypectomy surveillance, and how do their outcomes vary over long-term follow-up?In this cohort study of 59 667 patients (overall recurrence, 29.5%), high-grade dysplasia showed the strongest association with early recurrence, while villous histology demonstrated a late-phase (>10 years) resurgence. Sex displayed temporal heterogeneity with female patients with high-risk adenomas experiencing higher late-term (>10 years) recurrence risk than male patients. These findings suggest that both histopathologic and demographic factors show time-dependent associations with recurrence, supporting dynamic and individualized surveillance strategies.
