Can automated electroencephalographic (EEG) background analysis estimate 2-year neurodevelopmental outcomes in neonates with hypoxic-ischemic encephalopathy? In this cohort study of 203 neonates from the High-Dose Erythropoietin for Asphyxia and Encephalopathy trial, the Brain State of the Newborn score estimated neurodevelopmental outcomes, even after only 2 hours of recording. The Brain State of the Newborn score added information to clinical variables and was associated with similar prognostic accuracy as EEG background assessment by neurophysiologists. The findings suggest that early automated EEG background quantification may be a valuable tool for assessing early hypoxic-ischemic encephalopathy severity, identifying neonates for inclusion in studies of adjuvant neuroprotective strategies, and providing family counseling.
