Is ovarian morphology in women with polycystic ovary syndrome (PCOS) associated with long-term morbidity and mortality? In this cohort study with 37 years of prospective follow-up of 340 women with PCOS, those with polycystic ovarian morphology (PCOM) had significantly higher rates of non–insulin-dependent diabetes (23.8% vs 9.3%) than those without this pattern. No significant differences were found in overall mortality or other chronic diseases. This study found that PCOM in PCOS was associated with increased long-term risk for type 2 diabetes but not with increased mortality or broader chronic disease burden.
