What is the survival benefit of deceased donor kidney transplant compared with dialysis across different age groups and donor quality? In this cohort study of 8011 waitlisted patients from the Australia and New Zealand Dialysis and Transplant Registry, kidney transplants with a lower Kidney Donor Risk Index score were associated with a 6.6-month survival gain over 10 years, with the greatest benefits in older recipients. Transplants with a higher Kidney Donor Risk Index score offered smaller survival gains (3.6 months), with minimal benefits in younger recipients. The study results suggest that deceased donor kidney transplant is associated with improved survival compared with dialysis, but the magnitude of benefit varies depending on donor quality and recipient characteristics.
