Predicting Laryngeal Involvement in Pemphigus Vulgaris Using a Clinical Risk Score

Can clinical parameters predict deep laryngeal involvement in pemphigus vulgaris to guide laryngoscopy decisions?In this cohort study of 247 patients with pemphigus vulgaris, a scoring system using hoarseness, dysphagia, and disease severity achieved an area under the receiver operating characteristic curve of 0.779, with a 92.6% negative predictive value. Patients scoring 0 to 1 points had 0% laryngeal involvement, while those scoring 4 points or higher had a 66.7% negative predictive value.This scoring system may identify low-risk patients who can safely defer laryngoscopy, reducing unnecessary procedures.
