Among US Medicare beneficiaries, how have sex-specific patterns and outcomes of transcatheter aortic valve implantation (TAVI) evolved from 2013 to 2022?In this cohort study of 314 123 patients, the proportion of women undergoing TAVI declined from 47.6% to 43.6% over 10 years. Although periprocedural mortality and complications decreased substantially in both sexes, women consistently exhibited higher early risk but modestly lower long-term mortality than men. These findings suggest that despite substantial improvements in procedural safety, women continue to face higher early risks yet demonstrate modestly better long-term survival, emphasizing persistent sex-based disparities that warrant focused optimization of care.
