Clinical Decisions and Outcomes After Switching High-Sensitivity Cardiac Troponin Assays in Suspected ACS

What are the clinical and health system–level implications of changing high-sensitivity cardiac troponin assays?In this interrupted time-series study including 25 849 patients, switching cardiac troponin assays was associated with a doubling in the proportion of patients with suspected acute coronary syndrome identified with myocardial injury, more hospital admissions, and more serial cardiac troponin measurements without evidence of improved cardiovascular outcomes at 1 year.High-sensitivity cardiac troponin I and T assays are not equivalent; health systems and clinicians should be aware that switching cardiac troponin assays may impact hospital admissions, testing, and resource utilization.
