Physical Activity and Cardiovascular Outcomes in Phenotype-Negative Cardiomyopathy Variant Carriers
JAMA Cardiology 10.1001/jamacardio.2025.4466November 08, 2025 at 2:45 PM EST
Do the cardiovascular effects of moderate to vigorous physical activity (MVPA) on cardiovascular (CV) outcomes differ between individuals who carry a cardiomyopathy-associated variant with no known clinical manifestations (G+P−) and noncarriers? In this cohort study including nearly 4000 G+P− carriers with prospectively measured activity, greater MVPA volumes were associated with lower risks of atrial fibrillation, heart failure, myocardial infarction, stroke, and cardiomyopathy without increased risk of ventricular arrhythmia. Moreover, MVPA was associated with cardiac remodeling in G+P− carriers mirroring physiologic changes seen in noncarriers. The findings support the appropriateness of guideline-based MVPA recommendations for G+P− carriers.
