JAMA 10.1001/jama.2026.21737October 22, 2026 at 7:15 PM EDT
A Canadian matched cohort study of 385 pregnancies among 252 living kidney donors in Ontario, British Columbia, and Alberta, and 1,540 matched pregnancies among nondonors, found that pregnancy after kidney donation was associated with a higher risk of gestational hypertension, preeclampsia, or eclampsia (11.9% vs 8.3%). Donor pregnancies were also more likely to result in infants who were small for gestational age (13.8% vs 7.9%). Most pregnancies after kidney donation did not have serious complications.
