Digital Outreach to Improve Statin Refills in Patients With Low Statin Adherence

Among adults with a statin indication and recent statin nonadherence, adaptive outreach improved short-term refill vs usual communication. A second outreach produced additional gains among initial nonresponders, although switching communication modalities provided no additional benefit. These findings support adaptive, sequential health system outreach strategies to improve cardiovascular prevention when integrated into population health programs.
In this randomized trial of 20,604 adults with established ASCVD or high ASCVD risk who had fallen behind on their prescriptions, digital outreach modestly improved short-term statin refill compared with usual communication. A second reminder further improved refill rates among initial nonresponders, while switching to a different type of message offered no additional benefit. Among the intervention group, 24.9% refilled their statin by 28 days; among the control group, 21.2% refilled. The study was conducted within Kaiser Permanente Northern California.
