Immunotherapy Response in Microsatellite Instability–High Gastroesophageal Adenocarcinoma

What are the outcomes and durability of response to immune checkpoint inhibitor (ICI) therapy in patients with gastroesophageal adenocarcinoma (GEA) with deficient mismatch repair (dMMR) or microsatellite instability–high (MSI-H)?In this cohort study of 1638 patients with GEA who underwent MSI testing, 83 (5.1% of tested patients) had dMMR tumors, and ICI therapy was associated with high rates of best objective response in metastatic disease and clinical or pathological complete response in locoregional disease.ICI-based therapy was associated with high response rates in both metastatic and locoregional dMMR GEA in this study, suggesting the potential for nonoperative management in selected patients.
