Radiotherapy With a 12-Gene Expression Assay for Ductal Carcinoma In Situ

Among women with ductal carcinoma in situ (DCIS), can DCIS score (DS) profiling allow selective radiotherapy use after breast-saving surgery while maintaining low ipsilateral in-breast recurrence rates?In this single-arm clinical trial of 339 women with DCIS, patients underwent breast magnetic resonance imaging and surgery, and of those who underwent wide local excision (171 [50%]), 159 (93%) accepted omission or receipt of radiotherapy based on a DS of less than 39 or 40 or greater. The 5-year ipsilateral in-breast event rate was approximately 1% annually for the low and high-intermediate DS groups.The study findings suggest that DS-based selective radiotherapy use is acceptable for patients with DCIS, allowing half the population to avoid radiotherapy while maintaining a low risk of in-breast events for all.
