What is the accuracy of the plasma phosphorylated tau 217 (p-tau217)/amyloid-β 42 (Aβ42) ratio test for detecting amyloid–positron emission tomography (PET) positivity for an Alzheimer disease (AD) diagnosis in older veterans with and without a history of traumatic brain injury (TBI)?In this cross-sectional cohort study of a diagnostic test including 272 participants, the accuracy of the plasma p-tau217/Aβ42 ratio test was high in veterans without TBI history but was significantly lower in veterans with a TBI history, driven primarily by lower sensitivity.Study results suggest that TBI history and severity may act as modifiers of AD blood test accuracy in the prediction of brain amyloid-PET positivity.
