Intravenous Thrombolysis Use in the Late Time Window Before Interhospital Transfer for Thrombectomy

Does intravenous thrombolysis before interhospital transfer improve outcomes in patients with acute ischemic stroke with large vessel occlusion admitted to nonthrombectomy centers beyond 4.5 hours from the last time seen well?In this multicenter cohort study of 584 patients, intravenous thrombolysis prior to transfer was associated with better 3-month functional outcomes and higher recanalization rates during transfer, without safety concerns.These results suggest a potential benefit of late-window intravenous thrombolysis before transfer for thrombectomy and support the rationale for clinical trials in this setting.
