Are comprehensive smoke-free laws associated with long-term reductions in cardiovascular disease (CVD) mortality in US counties? In this comparative effectiveness research study using the generalized synthetic control method with staggered policy adoption, comprehensive smoke-free laws were associated with a mean reduction of 12 CVD deaths per 100 000 population annually over 12 postpolicy years, with larger reductions observed among adults aged 65 years or older, male individuals, and non-Hispanic White populations. These findings suggest that populations in areas with comprehensive smoke-free laws experience reductions in CVD mortality, although this association varied across subpopulations.
