Does a structured multidomain lifestyle intervention vs an education intervention modify trajectories of cerebrovascular magnetic resonance imaging (MRI) markers, and does any effect differ by age, in adults 60 to 79 years of age at increased risk for cognitive decline?In this prespecified secondary analysis of a randomized clinical trial of 959 participants, the structured intervention significantly attenuated increases in diffusion MRI–derived free water among participants younger than 70 years but not among those 70 years or older.Lifestyle interventions may preferentially influence early, microstructural cerebrovascular injury, supporting diffusion MRI free water as a sensitive marker and suggesting greater benefit when interventions are implemented earlier in late life.
