Use and Implications of the Fracture Risk Assessment Tool in Primary Hyperparathyroidism

How can the Fracture Risk Assessment Tool (FRAX) be used for patients with primary hyperparathyroidism to guide surgical decision-making?In this cohort study of 59 194 patients with primary hyperparathyroidism, observed major osteoporotic fracture and hip fracture events were slightly greater than estimated by FRAX. Compared with nonsurgical management, parathyroidectomy was associated with consistent reduction in major osteoporotic fracture above major osteoporotic fracture scores of 1.2% and hip fracture reduction above hip scores of 2.7%.These findings suggest that FRAX can stratify fracture benefit from parathyroidectomy and identify a larger subset of patients who may benefit from surgical management than considered by current guidelines.
