Physiologic Transition During Delayed Cord Clamping With Assisted Ventilation in Preterm Infants
JAMA Network Open 10.1001/jamanetworkopen.2025.45258November 24, 2025 at 11:00 AM EST
Is assisted ventilation during delayed cord clamping (DCC) associated with diffrences in physiologic transition after birth in extremely preterm infants? In this secondary analysis including 570 infants from the VentFirst randomized clinical trial of infants born at less than 29 weeks’ gestation, assisted ventilation during DCC was associated with significantly reduced odds of intubation in the delivery room for infants not breathing well 30 seconds after birth. For infants breathing well at 30 seconds, assisted ventilation during DCC was not associated with rates of intubation in the delivery room. These findings suggest that assisted ventilation during DCC may improve physiologic transition, particularly for infants with respiratory depression.
