What is the cost-effectiveness of mailed human papillomavirus (HPV) self-sampling kits (direct-to-all or opt-in) relative to usual care for increasing cervical cancer screening among members of a US health care system with varying screening histories? In this economic evaluation of 31 355 participants from a randomized clinical trial, the direct mailing of HPV kits was highly cost-effective among cervical cancer screening adherent and overdue members as was an opt-in approach among members, with unknown screening histories. These findings suggest that within US-based private integrated health care systems, directly mailing HPV kits for cervical cancer screening is cost-effective and affordable.
