Does circulating tumor DNA (ctDNA) correlate with next-generation sequencing (NGS) of tissue samples in patients with pancreaticobiliary tumors, and can it help estimate treatment response? In this cohort study with 790 patients, ctDNA testing significantly correlated with tissue NGS in finding actionable alterations for patients with pancreatic ductal adenocarcinoma and cholangiocarcinoma. Additionally, serial ctDNA testing found markers associated with higher odds of progressive disease. The findings of this study suggest that ctDNA testing can be used when tissue specimens are insufficient for analysis and can be complementary to tissue-based testing; ctDNA could also act as a marker of response and identify resistance mechanisms.
