Is cognitive behavioral therapy for insomnia (CBT-I) effective and acceptable for treating insomnia in individuals with chronic disease? This systematic review and meta-analysis of 67 randomized clinical trials including 5232 participants found that CBT-I was significantly associated with improved insomnia severity, sleep efficiency, and sleep onset latency with moderate to large effect sizes. Treatment associations were moderated by a variety of sample, treatment, and methodological factors, and treatment acceptability and satisfaction were high. These findings suggest that CBT-I is an acceptable and efficacious intervention for managing insomnia in chronic disease populations, supporting its use as a first-line treatment across diverse patient groups.
