Burst Exercise Stress Testing in Catecholaminergic Polymorphic Ventricular Tachycardia

Does the Burst exercise stress test (EST) increase the diagnostic and therapeutic yield of catecholaminergic polymorphic ventricular tachycardia (CPVT) compared with the traditional Bruce protocol?In this 2-center cohort study of 28 patients with CPVT, the Burst protocol was associated with more severe ventricular arrhythmias in 20 of 28 patients (71%) and a higher median arrhythmia score than the Bruce protocol (3 vs 1). These findings prompted β-blocker or flecainide initiation or dose escalation in 13 of 20 patients (65%), with the Burst protocol demonstrating no safety concerns.The Burst EST enhances detection of CPVT-related arrhythmias and improves individualized risk stratification and therapy adjustment, supporting integration into diagnostic and follow-up care.
