Trajectories of Physical Activity Before and After Cardiovascular Disease Events in CARDIA Participants

How do levels of moderate to vigorous–intensity physical activity (MVPA) change across adulthood and around cardiovascular disease (CVD) events, and how do trajectories vary by subgroup?In this cohort and nested case-control study among participants in the CARDIA (Coronary Artery Risk Development in Young Adults) study, MVPA declined steadily from young adulthood to midlife, then plateaued; CVD cases experienced a steeper decline beginning approximately 12 years pre-event, accelerating 2 years before, with a persistent postevent gap vs controls. Black women had the lowest MVPA throughout adulthood and the highest risk of low MVPA post-CVD.Findings highlight the need for strategies to address MVPA differences across the life course and promote recovery after CVD.
