Adjunctive Intra-Arterial Alteplase After Successful Thrombectomy for Acute Ischemic Stroke

Among patients with acute ischemic stroke due to large vessel occlusion who had successful recanalization after endovascular thrombectomy, does adjunctive intra-arterial alteplase improve functional outcomes compared with thrombectomy alone?In this randomized, open-label, multicenter clinical trial including 440 patients, adjunctive intra-arterial alteplase after successful thrombectomy resulted in a higher proportion of patients achieving excellent functional outcome (modified Rankin Scale score of 0 or 1) at 90 days compared with thrombectomy alone (57.5% vs 42.5%), a statistically significant difference.Adjunctive intra-arterial alteplase after successful thrombectomy may improve functional recovery in patients with large vessel occlusion stroke.
