YEARS Algorithm for Diagnosis of Suspected Pulmonary Embolism in Patients With Cancer

Does the YEARS diagnostic algorithm safely and efficiently rule out acute pulmonary embolism (PE) in patients with active cancer?In this randomized clinical noninferiority trial, the YEARS algorithm’s diagnostic failure rate was 1.8% compared with 5.5% with computed tomographic pulmonary angiography (CTPA) only, for an absolute risk difference of −3.7% with the upper boundary of the confidence interval not exceeding the noninferiority margin of 2.6%. Using the YEARS algorithm obviated the need to perform CTPAs in 22% of patients.The YEARS algorithm is as safe as a CTPA-only approach to exclude PE in patients with active cancer and avoids CTPA scans.
