Does individualized perioperative blood pressure management based on preoperative nighttime mean arterial pressure (MAP) improve outcomes in patients undergoing abdominal surgery who are considered at high risk of postoperative complications? In this randomized clinical trial that included 1272 participants, the incidence of a composite primary outcome (acute kidney injury, acute myocardial injury, nonfatal cardiac arrest, or death within the first 7 postoperative days) was not significantly different between patients assigned to individualized MAP targets and patients assigned to routine blood pressure management with a MAP target of 65 mm Hg or higher. This trial does not support individualizing blood pressure targets based on preoperative nighttime MAP in high-risk patients undergoing abdominal surgery.
