Risk of Pseudophakic Cystoid Macular Edema in Eyes With Previous Pars Plana Vitrectomy

Is a history of pars plana vitrectomy (PPV) associated with an increased risk of cystoid macular edema (CME) following cataract surgery? In this cohort study of 615 983 patients undergoing cataract surgery, including 7318 propensity score–matched pairs, CME occurred in 336 of 7318 patients with history of PPV (4.59%) compared with 90 of 7318 (1.23%) without PPV. The elevated risk persisted after excluding surgical complications. While numerous factors limit determining the clinical relevance of these findings, these results support the possibility that prior vitrectomy is associated with a higher risk of postoperative CME after cataract surgery and may warrant closer monitoring and enhanced anti-inflammatory prophylaxis.
