Pathologic Myopia Globe Shape and Long-Term Prognosis

Do baseline 3-dimensional (3D) eye shape subtypes defined by magnetic resonance imaging (MRI) predict 15-year structural and functional prognosis in highly myopic eyes?In this cohort study, 6 distinct prognostic trajectories were identified, delineating a morphologic map of the natural history of high myopia. Nasally distorted eyes constituted the highest-risk phenotype, exhibiting faster axial elongation and choroidal thinning and substantially increased risks of myopic macular degeneration progression, visual acuity loss, and field defects.3D-MRI–based eye shape classification enables long-term prognostic stratification in high myopia, potentially guiding personalized surveillance and early intervention to prevent vision loss.
