A 10-Year Cost-Effectiveness Analysis of Metabolic and Bariatric Surgery in Adolescents

Is metabolic and bariatric surgery (MBS), utilizing sleeve gastrectomy or Roux-en-Y gastric bypass (RYGB), cost-effective for adolescents with severe obesity over a 10-year horizon?In this economic evaluation of 100 000 simulated adolescents with severe obesity, both RYGB and sleeve gastrectomy were estimated to be cost-effective compared with no surgery alone, with incremental cost-effectiveness ratios of $50 271 and $41 164 per quality-adjusted life-year gained, respectively. However, RYGB was not cost-effective when directly compared with sleeve gastrectomy.These results suggest that MBS is a cost-effective long-term treatment strategy for adolescents with severe obesity, supporting broader use in clinical care and policy decisions.
