Oral Corticosteroids and Adverse Events in Chronic Rhinosinusitis With Nasal Polyps

Among patients with chronic rhinosinusitis with nasal polyps (CRSwNP), is oral corticosteroid (OCS) exposure associated with increased long-term systemic adverse event (AE) risk and what upper limit on exposure should be used to minimize AEs?In this nested case-control study of 523 316 patients (51 647 case patients and 472 369 matched controls), high exposure intensity (>90 days per year) and high cumulative dose (>1.0 g per year) were associated with an increased risk of systemic AEs, in particular, avascular bone necrosis, osteoporosis, and pneumonia.These findings suggest that cumulative OCS exposure should be minimized in patients with CRSwNP, supporting a shift toward steroid-sparing strategies for the long-term management of CRSwNP.
