Are plasma Alzheimer disease biomarkers associated with postoperative delirium (POD) risk in older adults undergoing elective cardiac surgery? In this cohort study of 491 patients, elevated plasma amyloid-β 1-40 (Aβ1-40) levels were significantly associated with higher POD risk, independent of age and baseline cognition. Multivariable models combining Aβ1-40 with clinical factors and Montreal Cognitive Assessment showed improved risk estimation of POD compared with clinical variables alone. These findings suggest that preoperative plasma Aβ1-40 may serve as a modest but independent risk factor for POD in older patients undergoing cardiac surgery, offering potential value for future multimodal risk stratification.
