Cost-Effectiveness of Fecal Immunochemical Testing Alone vs Co-Testing With Stool Antigen

Among individuals invited to undergoing colorectal cancer screening with a fecal immunochemical test (FIT), is the addition of a stool antigen test cost-effective compared with FIT alone?In this economic evaluation based on a randomized clinical trial involving 240 000 individuals, invitation to co-testing for stool antigen in addition to FIT was cost-saving in 65.7% of 100 000 probabilistic simulations and cost-effective in the remaining 34.3% compared with FIT alone.In a pragmatic randomized comparison of invitation strategies, inviting individuals to undergo combined stool antigen testing and FIT was more cost-effective than inviting FIT alone when evaluated under real-world conditions with incomplete adherence.
